  7. DBA or Fictitious Name Certificate (if applicable)
  8. Operating Agreement, By-Laws, Partnership, or Trust Agreement (if applicable)
  9. Franchise Agreement or Fuel Supply Contract (if applicable)

  1. Full Unredacted Filed Tax Returns – For the three years preceding the date of the loan request.  
  2. Personal Identification – Front and back image of valid and unexpired state driver’s license or state identification.  
  3. If a 20% or more owner is not a U.S. citizen, but is a lawful permanent resident, the following are required:   
    1. Copy of Permanent Resident Card (front and back)   
    2. USCIS Verification & Authorization Letter  
      • Note: Please note that once your application is submitted, your assigned Relationship Manager will provide you with the USCIS Verification & Authorization Letter for you to complete. Please physically sign this form with blue ink and return to your assigned Relationship Manager
